Seasonal Space Request Instructions and Online Application
The online application to request bulk Facility Space in the City of burlington has been automated for ease of use and to ensure all required information is communicated to the Parks and Recreation Space Allocation Team. This will assist us in processing your application in a timely manner.
Important notes before you begin filling out your application(s).
- Each application form will be for a specific Type, Level and Age group (example hockey, rep, youth/Teen).
- Each application form will allow for 10 booking requests. For example one booking request would be: Appleby Arena, Wednesdays, 8pm-10pm. If you require more than 10 bookings, you will need to fill out additional application forms. Example: 56 bookings will require 6 application forms.
- Please ensure all required fields are filled out, otherwise this could cause delays in processing your application.
- Once you have completed an application form, click “Submit” and you will receive a message confirming your application has been submitted. Please print a copy for you records.

· Section C Booking (1-10)
· Section D Additional Information
3. Review your form to ensure accuracy of information.
4. Submit your application.
5. Print confirmation message.
