By submitting this application form you are applying to nominate yourself or another property owner(s) who has by means of maintenance, renovation or restoration, contributed to the preservation of our built heritage.   Entries must be received by December 31, 2008.

Personal information on this form is collected under the authority of the Municipal Act, S.O. 2001, c.25 and will be used to determine the suitability of awarding the Burlington Built Heritage Awards.  The photographs submitted as part of the award process, including names and addresses of nominated properties and owners, will be publicized in local newspapers and recognized during the Annual Heritage Awards Evening.  Inquiries may be directed to the attention of the City Clerk:
