Q. How would I obtain an Anniversary Certificate?
A. Contact the Mayor's office by calling 905-335-7607 or email mayor@burlington.ca
Please provide the following information:
- Name of the couple Address of couple (include postal code)
- Year of Anniversary (i.e. 50th, 60th, etc.)
- Date of Anniversary
- Contact person - this person will be contacted when the certificate is completed
- Contact's phone number
Please allow up to 4 weeks for certificate to be prepared.
Q. How would I obtain a Birthday Certificate?
A. Contact the Mayor's office by calling 905-335-7607 or email mayor@burlington.ca
Please provide the following information:
- Name of the person
- Address of the person (include postal code)
- Year of Birthday (i.e. 80th, 85th, etc.)
- Date of Birthday
- Contact person - this person will be contacted when the certificate is completed
- Contact's phone number

Q. How would I obtain a Proclamation?

Please provide the following information:
- Name of Organization
- Address of Organization (include postal code)
- Phone Number of Organization
- Name of Proclamation
- Date and duration of proclamation (i.e. mm/dd/yy OR mm/dd-dd/yy)
- Provide wording for proclamation (i.e. Whereas the...)
Please allow 4 weeks for proclamation to be prepared.
Criteria: Proclamations will be included in the Council agenda only if they meet all of the following criteria:
- Shall be submitted by representatives of a recognized charity with offices located in Burlington or by a community group based in Burlington;
- Will be issued only in respect of activities that support residents of the City of Burlington or related events taking place in Burlington;
- Must demonstrate respect and tolerance for all Burlington residents; and
- Should foster a sense of community; and
- Should not be used as a vehicle to attempt to influence government policy.
Individuals and institutions seeking proclamations that recognize regional, provincial or national causes or events will be referred to the appropriate other order of government for recognition.
Q. How would I request a Flag Raising at City Hall?

Please provide the following information:
- Name of Organization
- Contact person
- Address of Organization (include postal code)
- Phone Number of Organization
- Reason for Flag Raising
- Advise if the Organization is to supply their own flag or if the
- City of Burlington has one on file. Please allow 4 weeks for the Flag Raising to be scheduled.
Q. How would I request a Civic Recognition Award?

Please provide the following information:
- **Type of award(s) (i.e. Provincial, National, International) the person or team has won
- Brief (1/2 page) summary of nominee's current accomplishment, past accomplishments, years of training, coaches, etc.
- Name of Team, Coaches, Managers, Players
- Please complete Civic Recognition Award Form and mail it to Mayor's Office, City Hall, 426 Brant Street, P.O. Box 5013, Burlington, ON L7R 3Z6
